Custom forms
Build your own forms with custom fields, add no-code workflow rules that flag submissions or raise CAPAs, and read every response on the responses page.
Custom forms let you build your own data-collection forms without a developer. Add the fields you need, save, and your team can start filling them in — and workflow rules can act on the answers.
Design a form
- Open Custom forms.
- Enter a form name and a category.
- Under Fields, add each field with a key, a label, a type (text, multiline text, number, checkbox, date, or email), and tick required if it's mandatory. Use + Add field for more.
- Click Create form. It appears in Your forms.
Add workflow rules (optional)
- Under Workflow rules, make the form act on an answer: pick a field, an operator (equals,
not equals, contains, greater than, less than), and a value, then choose an action:
- Flag for review marks the submission so it stands out on the responses page.
- Raise CAPA auto-creates a corrective action from the submission — give the rule a CAPA title (a response raises at most one CAPA even if several rules match).
- Rules are evaluated on every submission; see CAPA for what happens to raised actions.
Fill in a form
- In Your forms, click Fill next to a form, pick the site, answer the fields and Submit. Any workflow rules run against what's submitted.
Read the responses
- Click Responses next to a form (or go to
/forms/{id}/responses). - Every submission is listed newest first, showing who submitted it, when (UTC), and each answer under its field label.
- Use the Show filter to see only responses flagged for review — flagged ones carry a warning badge and border.
- A response whose rule raised a CAPA shows a badge linking straight to that CAPA, so you can follow the corrective action without leaving the trail.
Tip: keep field keys short and stable — the label is what people see, so put the friendly wording there. Workflow rules refer to fields by key.
