Observations (BBS)

Capture safe and at-risk behaviors, conditions, and near misses from the field — online or offline — classify SIF potential, and raise CAPA on the unsafe ones.

Behavior-based safety observations are quick field notes on what you saw — a safe behavior worth reinforcing, or an at-risk behavior or condition worth fixing. They're your best source of leading-indicator data.

Log an observation

  1. Go to Observations and use the Log an observation form at the bottom.
  2. Pick the site, the type (safe behavior, unsafe behavior, unsafe condition, near miss), and a category (PPE, body position, housekeeping, ergonomics, tools & equipment, procedures, other).
  3. Write a short description, choose who observed it from the roster picker, set the date, and note any immediate action taken.
  4. Press Log observation. It appears in the list right away.

Filter and close out

  • Filter the list by site and status; the Unsafe / at-risk filter surfaces items that need attention (the dashboard's unsafe-observations tile deep-links here pre-filtered).
  • Open an observation to see the detail and its tamper-evident audit trail, or Close it (optionally recording the action taken).

Classify SIF potential

On the observation detail page, answer the four energy-based questions (high energy present? released? serious injury? direct control in place?) and the observation gets an SCL category — separating potential serious-injury exposures from low-energy items.

Raise a CAPA

Open observations show a Raise CAPA button (list and detail page). Click it to open a corrective action linked back to the observation; a green CAPA badge then links straight to it.

Capture from the field — no login, no signal

The Public field-capture links page (/report-links) generates a no-login link per site, with a printable QR code. It opens an offline-capable form with a Log an observation mode (plus report-a-problem and stopped-work modes) — submissions made with no signal are queued on the device and sent automatically when connectivity returns. Tip: put the QR on a poster at the gate — anyone on the floor, including contractors, can log an observation in under two minutes.
