Constituent reports
Receive safety and incident reports filed up to the agency by its constituents — including anonymous public submissions via a shareable link — and triage each one from submitted through closed.
Constituents — member organizations, businesses, or the public — can file safety and incident reports up to
the agency. Agency oversight → Reports (/agency/reports) is the intake and triage queue. Any agency
oversight role can view and triage reports.
Where reports come from
- The public reporting link. Click Generate public link to mint a shareable URL (put it on a website or a QR code). Anyone can open it and file a report with no login — it lands here as Submitted. Reports can be anonymous; a name is optional.
- Record a report received. Log a report that came in another way (phone, email, in person) using the form at the bottom — choose the reporter (a member or external), the type, and a description.
Triage
Each report moves through a short workflow:
- Submitted — a new report, highlighted so it stands out in the queue.
- Start review — you're working it.
- Close — resolved.
Use Export CSV to download the reports register.
What happens automatically
- Public submissions are validated against a signed link for your agency — a report can only be filed to the agency the link was minted for, and the link can't be tampered with.
- Public submissions land as Submitted so nothing filed by a constituent is missed.
