Most safety programs measure themselves on things that have already gone wrong: the recordable rate, the lost-time count, the DART. Those numbers matter, but they share a flaw — they're lagging indicators. They tell you about injuries that already happened. By the time they move, someone's already hurt. The near-miss is the signal that points the other way, and most organizations barely collect it.

Leading vs. lagging, plainly

  • A lagging indicator counts outcomes after the fact: injuries, illnesses, incident rates. Essential for accountability, useless for prevention in the moment.
  • A leading indicator measures the conditions and behaviors that precede outcomes: inspections completed, hazards found and fixed, training current, and near-misses reported.

The whole point of a leading indicator is that it moves before the injury does — giving you a chance to act while it's still just a story about what almost happened.

Why the near-miss is special

A near-miss is a free lesson. Same hazard, same sequence, same failure — minus the injury, by luck. Every near-miss you capture is a hazard you can fix before it collects its due. Organizations that surface a lot of near-misses aren't more dangerous; they're more visible. They've traded the illusion of safety (a quiet incident log) for the reality of it (a steady stream of small fixes).

The catch: near-misses are, by definition, the events where nothing happened — so there's no wreck to investigate, no one off work, nothing forcing them into the record. If reporting one is any trouble at all, it simply won't happen.

Why most programs don't capture them

Near-miss reporting fails for predictable, human reasons:

  • Friction. If reporting means logging into a system, finding the right form, and filling fifteen fields, a busy worker won't do it for an event where nobody got hurt.
  • Fear. If reports feel like they invite blame, people stay quiet. Psychological safety is a prerequisite, not a nicety.
  • The void. If a report disappears and nothing visibly changes, people learn that reporting is pointless — and stop.

Fix all three and near-miss volume climbs. Leave any one and it flatlines.

What actually works

The pattern that produces near-miss reporting is the opposite of friction, blame, and silence:

  1. Make it take seconds. Capture from a phone in the field — ideally a QR poster anyone can scan, a few taps, no login, anonymous if they want it. If it takes longer than a smoke break, it's too long.
  2. Make it feel safe. Frame it as hazard-hunting, not fault-finding, and allow anonymous reports.
  3. Close the loop visibly. Route each report to an owner with a due date, drive the corrective action to closed, and let people see that their report led to a fix. Nothing fuels the next report like seeing the last one matter.
